You will have the project name, created by, and contact information to enter at the very top. If your project is associated with multiple pages, then you can list the document number and a version ID to the right of the page.
In the main table of the template, there are spaces to list the ID of the employee submitting the document, a description of the possible issue, and their “Risk Response” (how they intend to handle and solve the issue). If that solution requires a follow up by you or your team, then you can write something in the last space of the document.
